BEFORE this visit, please MAKE SURE to complete our New Patient Forms below and hit SUBMIT at the bottom of the page. ALL HIGHLIGHTED AREAS ARE REQUIRED. If this is not completed before your first visit, it could shorten your treatment time as we’ll need to submit these first.
